OBSERVED BUILD UP OF BLACK MOLD INSIDE ICE MACHINE. PIC TURNED ICE MACHINE OFF AND STATES THEY WILL CLEAN IT WHEN ALL THE ICE MELTS. WILL FOLLOW UP WITHIN 48 HOURS TO CONFIRM ICE MACHINE WAS PROPERLY CLEANED AND SANITIZED.
FOLLOW-UP INSPECTION WAS CONDUCTED ON 8/6/25 TO CONFIRM THE ICE MACHINE WAS PROPERLY CLEANED AND SANITIZED.

FOLLOWING A COMPLAINT ABOUT THE CEILING LEAKING FROM A/C UNIT AND CEILING BEING IN BAD SHAPE IT WAS OBSERVED TODAY TO BE LEAKING IN THE KITCHEN AREA. WHILE TALKING WITH THE PIC (SASHA CROSS) IT WAS THEN OBSERVED THAT WATER WAS LEAKING AROUND THE HOOD OVER THE FRYER STATION. SHE STATED SHE HAD NOT SEEN THAT BEFORE HOWEVER IT WAS RAINING OUTSIDE AND IT IS POSSIBLE THAT THE ISSUE IS COMING FROM OUTSIDE THE BUILDING. THE CEILING SHOULD BE MAINTAINED IN CLEAN WORKING ORDER. IF THE ISSUE IS NOT RESOLVED WITHIN A TIMELY MANNER IT COULD AFFECT THE SAFETY OF THE FOOD BEING SERVED IN THE FACILITY. POSSIBLE CONTAMINATION FROM A/C LINES OR OUTSIDE WATER SOURCES. FACILITY WILL BE GIVEN A 6-MONTH TIMELINE TO HAVE THE ISSUES RESOLVED.

OBSERVED SEVERAL ITEMS ON THE PIZZA PREP LINE LACKING PROPER COLD HOLDING TEMPERATURES. ITEMS INCLUDE; PIZZA SAUCE, PEPPERONI, CHICKEN CHUNKS, GROUND BEEF, SLICED HAM, AND DICED TOMATOES. PIC STATED THEY WOULD MOVE FOOD ITEMS TO THE WALK IN COOLER TO PROPERLY COOL AND BRING OUT NEW FOOD THAT IS AT PROPER TEMPERATURES.
