Observed tcs foods (milk and boiled eggs) above 41F without active time control at reach-in cooler. See temperature chart below. Corrective Action: CFSM was notified, per discussion with CFSM food items were within time parameters. Food items were moved to another reach-in cooler to rapidly cool. Discussed filling cooler with less food items, lowering cooler temperature and monitoring temperature of cooler to ensure it is able to maintain temperature of 41F or below.

Observed multiple milk containers with use by date of 09/30 and inspection performed on 10/02. Corrective Action: CFSM was notified, milks were discarded. Discussed using FIFO method.

Observed interior of microwave soiled with encrusted food debris. Corrective Action: CFSM was notified employee assigned to clean. Discussed increasing cleaning frequency of interior of microwave.

Observed egg masher stored as clean with egg residue on it. Corrective Action: CFSM was notified, employee assigned to clean egg masher to sight and touch.

Observed 3 compartment sink sanitizer not at proper minimum concentration per manufacturers recommendations. Corrective Action: PIC was notified, work order placed to have sanitizer line checked. Will perform informal inspection on 10/4 to verify issue has been resolved. The mechanical dishwasher is to be used until the 3 comp sink sanitizer is repaired and at proper minimum concentration per manufacturers recommendations.

Observed employee with no hair restraint. Corrective Action: PIC was notified, employee put on hair net. Discussed the need for a hair restraint at all times when working with food.
