Observed food service employee wash hands in prep sink. CA: Food employees shall clean their hands in a handwashing sink or approved automatic handwashing facility and may not clean their hands in a sink used for food preparation or warewashing sink. COS: Employee directed to wash hands in handwashing sink.
4-2A - food stored covered

Observed multiple containers with different foods in reach-in freezer uncovered. Food items were not actively cooling. CA: Food must be covered except when cooling. COS: Food containers were covered.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed meatballs in reach-in freezer with thaw date of 4/26 and inspection performed on 5/5. CA: Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition. A food that requires datemarking shall be discarded if it exceeds 7 days. COS: Meatballs were discarded.

Observed pan of lasagna thawing on counter top. CA: Time/temperature control for safety food shall be thawed under refrigeration that maintains the food temperature at 41°F (5°C) or less. COS: Pan of lasagna was below 41F, pan was placed back in freezer.

Observed plastic containers storing cut lettuce and other plastic containers chipped. CA: Utensils and food contact surfaces of materials shall be of a smooth and cleanable surface. COS: PIC discarded broken containers that are no longer of a smooth and cleanable surface.
