Observed prep to cutting board with dark stain. 511-6-1.05(7)(a)1 - Equipment, Food-Contact Surfaces,& Utensils. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC will replace cutting board.

Observed ice bin in front food service uncovered while employee standing next to the bin. 511-6-1.04(4)(z) - Miscellaneous Sources of Contamination (C) (z) Miscellaneous Sources of Contamination. Food shall be protected from contamination that may result from a factor or source not specified under paragraphs (a) through (y) of this subsection. PIC closed the bin.

(a) Good Repair. All physical facilities shall be maintained in good repair. PIC advised to place order for maintainance.

(a) Good Repair. All physical facilities shall be maintained in good repair. PIC will repair with 10 days.
17D - adequate ventilation and lighting; designated areas used

Observed in adequate light in walk-in cooler. 511-6-1.07(3)(f) - Lighting Intensity, adequate in food prep, storage & service areas (C) (f) Lighting Intensity. The light intensity shall be: At a surface where food is provided for consumer self-service such as buffets and salad bars or where fresh produce or packaged foods are sold or offered for consumption;
(ii) Inside equipment such as reach-in and under-counter refrigerators;
(iii) At a distance of 30 inches (75 cm) above the floor in areas used for handwashing, warewashing, and equipment and utensil storage, and in toilet rooms. PIC will install a brighter light.
