Observed establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. 511-6-1.03(6) - Clean-up of Vomiting and Diarrheal Events(Pf) (6) Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. PIC advised to purchase of put together the kit and procedural steps.

Observed gap in back door and protective barrier missing parts. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors. PIC will have door repaired. PIC advised to fixe within 5 days.

Observed current permit not posted and permit expired and not renewed. 511-6-1.02(1)(f)(1) - Permit, Displayed. Post the permit in a location in the food service establishment that is conspicuous to consumers . PIC received a legal notice for owing an outstanding fee and was given 4 days to clear her debt, renew her permit and have it posted in the food service.
