Observed the floors and walls in the kitchen need cleaning and repair. (CA) Advised the PIC to clean and repair the kitchen floors and walls. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Observed the permit for the facility had expired in January. Upon further inspection, EHS observed that the facility has not paid since last year.

1. Post the permit in a location in the food service establishment that is conspicuous to consumers
