Observed stains on linen in multiple rooms inspected. Observed in rooms 227, 233,337. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed excessive buildup of dirt and dust on filters in A/C unit. Observed in every room inspected other than 102 and 114

Observed buildup of mold/mildew inside the ice machine chute. The machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. C/A: increase frequency of cleaning
