Observed uncovered lemons and limes above the last fridge on the grill prep line.

An employee discarded them voluntarily at the EHS request.
Regulation: 511-6-1.05(6)(b) - cutting surfaces (c)
Observed all the cutting boards for the grill prep line were heavily scored with black mold like substances in the gashes that were not in use.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.
PIC will discard the cutting boards and replace them.

Observed several expired, unopened test strips in the facility. EHS tested the equipment to ensure the proper concentration levels.
A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Pf
PIC ordered new test strips for the facility.

Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed a brush and a blow torch being stored in the handsink.
A handwashing sink shall be maintained so that it is accessible at all times for employee use. Pf
A handwashing facility may not be used for purposes other than handwashing. Pf
EHS had PIC remove the brush and blow torch.
Regulation: 511-6-1.04(4)(q) - food storage (c)
Observed a tub of fries stored on the floor of the kitchen.
(i) In a clean, dry location;
(ii) Where it is not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 cm) above the floor.
EHS had an employee move the fries off of the floor.

Observed no hot water at any of the sink in the facility. Upon further investigation, EHS observed an issue with the water heater in the right corner of the kitchen next to the hand sink.
PIC will contact a plumber to service the water heater.
Regulation: 511-6-1.06(5)(o),(p) - using drain plugs; maintaining refuse areas (c)
Observed food debris, plastic debris, a broken chair and a pile of fresh poop in the dumpster area. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. PIC will have the refuse area cleaned.

Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed several TCS items that above 41F in the WIC. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. PIC will discard TCS items and contact vendor to service the WIC.

Observed several TCS items through out the that were uncovered and not actively cooling. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; PIC will ensure that all TCS food items are covered and protect from over head contamination.

Observed several soiled cloths in the grill prep area . 4. Dry wiping cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be free of food debris and visible soil. PIC will ensure upon being soiled that cloth will be changed out to prevent contamination.
