The glasses in the guestroom are soiled and have been washed in the bathroom sinks. COS The glasses will be washed in the three compartment sink in the hotel kitchen. Advised PIC that the glasses shall be washed, rinsed and sanitized in a 3-compartment warewashing sink

Observed mildew like substance on the shower wall in several of the guestrooms. Advised PIC that toilet rooms and fixtures shall be kept clean and in good repair

Advised PIC water glasses must be clean and sanitized to sight and touch and air-dried only. Suggested offering single-service cups in lieu of water glasses

Carpets contained food debris, stains, and particles of trash. A/C filters dirty. Advise PIC to have housekeeping vacuum and clean A/C filters in each room after each guest stay, and at a minimum of once a week for stayovers

Water glasses located in all guest rooms were soiled with soiled, had fingerprints, and obvious towel fibers from drying. Advised PIC all water glasses must be clean to sight and touch and must be air dried only. Suggested offering single-service cups in lieu of water glasses

Carpets contained food debris, stains, and particles of trash; Curtain in room 100 was stained with tan-colored food-like debris; Microwave plates and button pads in several rooms were soiled with food. Advise PIC to clean and vacuum carpets, clean curtains, and clean microwaves to sight and touch

Yellow stains on pillow case in room 100, the sofa bed bedding was not replaced and had stains and human hair in room 218, and the sofa bed had a large yellow stain located in the center of the mattress in room 412. Advise PIC to remove bedding stains or replace bedding with new, change all bedding in guest room after each guest stay, and replace stained mattresses as often as necessary

Most current inspection report was not posted. Advise PIC to keep most current TA inspection report posted conspicuously for guests to view

All bathing/shower chairs were soiled with pink mold-like substance and dark accumulations in all crevices, and pink mold-like substance in several shower crevices discoloring the silicone-type seals. Advise PIC to clean and sanitize shower enclosures and chairs to be free of mold-like substances and accumulations

PIC could only show last 6 months of monthly pest control service records. Advise PIC to keep no less than 18 months of pest control records on file in facility to be shown during subsequent inspections

Observed dark, mold-like substance coating the blower vents and bottom of filters of the air conditioning unit in room 218. Advise PIC to clean and sanitize A/C unit to be free of any mold-like substances
