The glasses in the guestroom are soiled and have been washed in the bathroom sinks. COS The glasses will be washed in the three compartment sink in the hotel kitchen. Advised PIC that the glasses shall be washed, rinsed and sanitized in a 3-compartment warewashing sink
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Inspector observed notable yellow stains on mattresses, sofa mattresses, and mattress covers in guest rooms. Corrective action: clean and remove stains from mattresses and coverings between guest stays or replace the stained mattresses/mattress covers with new
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed mildew like substance on the shower wall in several of the guestrooms. Advised PIC that toilet rooms and fixtures shall be kept clean and in good repair

Observed soiled/stained carpet in several guestrooms and in the hallways throughout the hotel. Advised PIC that the carpet shall be maintained in good repair and clean in the guestrooms and hallways/common areas throughout the hotel
15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean

Inspector observed mold-like growth on FCS of ice machines. Corrective action: clean and sanitize FCS of ice machines at a frequency to preclude mold-like accumulations
