Inspector observed "Cheese Dipping Sauce" cups (with manufacture labeled "Perishable Keep Refrigerated") measured greater than 41 degrees F (Location: reach-in cooler under the prep table, nearest exit of pizza oven). COS PIC discarded. Advised PIC to cold-hold TCS items at 41 degrees F or below.

Inspector observed facility without written procedures for bodily fluid clean-up. Inspector observed facility without disinfectant & supplies for bodily fluid clean-up. CA: Facility will obtain written procedures, disinfectant, and supplies within 48hours.
