Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed the front hand sink near the drive thru window with hot water less than 85F. Corrective action: repair/replace faucet or the adjust the thermostatic mixing value beneath the sink to provide a minimum of 85F hot water to the hand sink at all times.
2-2E - response procedures for vomiting & diarrheal events
The facility does not have a biohazard clean up kit. Corrective action: obtain a vomit and diarrheal event clean-up kit effective for use against norovirus.
16C - sewage and waste water properly disposed
Regulation: 511-6-1.06(4)(h),(i) - approved sewage disposal; system; other liquid wastes and rainwater (p) (c)
Observed the walk in cooler drain line in disrepair, broken near the area where it passes through the walk-in cooler wall to the drain outside the unit. Corrective action: repair/replace the drain line in the walk in cooler

Regulation: 511-6-1.05(6)(q)1&3 - good repair & calibration (c)
Observed the sandwich prep cooler in disrepair and the interior of the unit with a buildup of mold like debris in the interior of the bottom of the unit.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Observed used and or equipment in disrepair, the second 2 well fry station and 2 door refrigeration in the dish pit area, and sandwich prep unit. Corrective action: repair or remove equipment.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(6)(h),(i) - warewashing equipment, clean solutions; manual warewashing equipment, wash solution temperature (c,pf)
Observed the sanitizer dispenser line is not long enough to reach the third compartment (sanitization sink) of the three-compartment sink. Corrective action: repair replace hose to reach the third compartment.
15C - nonfood-contact surfaces clean
Observed the speed racks in the walk in cooler with a build up of debris on the shelves. Corrective action: clean with a frequency that prevents build up.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the floors under the fry stations and batter line with a build up of food and debris beneath equipment. Corrective action: clean with a frequency that prevents a build up of debris.
