Observed broken faceplate and missing filter screen on the AC unit located in the hallway on the third floor. Observed damaged filter screen and missing filter screen on the AC unit located in the hallway on the second floor. CA: All furnishings must be maintained in a state of good repair. COS: No COS noted at this time. The facility will have the damaged/ missing filter screens and faceplate repaired/ replaced. This will be rechecked by the Health Department
