Observed a metal plate and buildup of dust in cabinet of room 220, and dirty microwave in room 320. CA: PIC assigned housekeeping to reclean

Observed non-operational telephones in room 220 and 316. CA: PIC will repair or replace phones. Inspector will return to recheck

Observed dust build-up on A/C vent screens and bathroom vents in rooms 210, 229, 327, and 315. CA: PIC will assign housekeeping to clean A/C vent screens and bathroom vents. Inspector will return to recheck

Observed bathtub in room 124 without anti slip surface. CA: Discussed placing anti slip mat/strips with PIC. PIC will add anti slip surface to bathtub

Observed smoke detector in room 320 non-operational. CA: PIC will have smoke alarm batteries changed.

Observed room 319 with non-operational phone. CA: PIC will have phone fixed

Observed rooms 111 and 318 with nonoperational phone. CA: PIC will have phones serviced and repaired

Observed smoke detectors in room 324 non-operational. CA: PIC will have smoke alarm batteries changed and test alarm

Observed room 215 with nonoperational phone. CA: PIC will have phone serviced and repaired

Observed personal items left in drawer in room 214. CA: PIC removed items from room

Observed room 317 without a non-slip mat in the bathtub. CA: PIC has new slip mats ordered and will place into bathtubs

Observed food buildup on interior surfaces of microwaves in rooms 116 and 120. Corrective Action: PIC was notified. House keeping staff was assigned to clean and sanitize microwaves in these rooms prior to renting

Observed smoke detectors not operating properly in rooms 120, 223, and 312. Corrective Action: PIC was notified. These rooms will not be rented until all smoke detectors have been repaired or replaced. This will be rechecked by Health Dept

Observed chewing gum on box springs in room 120. Corrective Action: PIC was notified. Housekeeping staff was assigned to remove chewing gum prior to renting this room

Observed heavy black buildup on customer ice machines at 1st and 3rd floor. Corrective Action: PIC was notified. Ice machines will be placed out of order. House keeping staff will be assigned to clean and sanitize ice machines. This will be rechecked by Heath Dept prior to use

Observed mattress cover with large stain in room #229 Corrective Action: PIC was notified. Housekeeping staff was assigned to change mattress cover onsite prior to renting to many guests

Observed interior hotel door to pool area not latching and locking properly. Corrective Action: PIC was notified. Work order will be submitted for immediate repairs. Employees were instructed to keep this door closed and locked at all times until repaired and rechecked by Health Dept. Guest must access pool from exterior gate
