Observation: Observed residue accumulation on the interior of the refrigerator in room 309. The refrigerator was observed to be unplugged.

Corrective Action: Discussed cleaning frequency of furnishings with the PIC. The PIC plugged the refrigerator in and had an employee clean the interior

Observation: Observed an accumulation of dust/ debris on the AC vent screens in room 303.

Corrective Action: Discussed cleaning frequency of AC vent screens with the PIC. The PIC had an employee clean the vent screens and will increase cleaning frequency

Observation: Observed plastic food storage container in the refrigerator of room 112. Per the PIC, the container had been left in the refrigerator by a previous guest.

Corrective Action: Discussed the cleaning frequency of refrigerators with the PIC. The PIC removed the plastic food storage container and will educate staff on cleaning frequency

Observed drawer in room 219 with missing handle. CA: PIC will have handle replaced

Observed to go plates of food left in mini refrigerator in room 311. CA: PIC removed plates to be thrown away and had room serviced for cleaning

Observed crack in bottom of the bath tub in guest room 207. Corrective Action: PIC notified. Discussed requirement for all fixtures to be smooth, nonabsorbent and shall be kept clean and in good repair. This room will be placed out of service until bath tub has been repaired or replaced. This will be rechecked by the Health Department

Observed mechanical ventilation fan in guest room 207 not turning on. Corrective Action: PIC was notified. Maintenance will repair. This room will be placed out of service and will not be rented until it has proper ventilation

Observed peeling paint and seam from the ceiling and wall above the shower in guest room 104. Observed small areas of black build-up within the peeling seam. Per PIC, this was from a previous leak that was repaired. Corrective Action: Discussed requirement for all floors, walls and ceiling to be of sound construction, properly maintained in good repair and kept clean. This room will be placed out service until the ceiling and wall are properly repaired

Observed stained and damaged ceiling tiles on ceiling in ice machine area of 2nd floor from past leak. Corrective Action: Discussed requirement for ceilings to be clean and in good repair. The damaged and stained ceiling tiles will be replaced. This will be rechecked by the Health Department

Observed black build up in both guest ice machines (1st and 2nd floors). Corrective Action: Discussed with PIC. Ice machine will be cleaned and sanitized immediately, and frequency of routine cleaning will be increased

Observed unlabeled chemicals in spray bottle and jugs in laundry area. Corrective Action: Discussed properly labeling bottles of chemicals with their common name for identification when not in the original container. PIC will have all bottles properly labeled
