Observed residential pesticides in the dry storage closet/office room in the back. COS. Discarded. Code: Chemicals shall be used according to their conditions of use, including using residential pesticides only in residential settings.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(i) - service sinks, numbers and capacities (c)
Observed that when the facility was moved from septic to sewer that the contractors had to remove a portion of the flooring under the mop sink to make the change and the mop sink/service sink was never re-installed. Code: Facility shall have at least one service sink or curbed service sink.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed accumulation of debris on non-food contact surfaces, particularly on the top of shelves. Code: Equipment shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.

Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Observed unsealed concrete as the floor in the corner by the water heater. Code: Floors, walls, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(c) - floor and wall junctures, coved, & enclosed or sealed, water-flushed floors provided with drains & graded to drain (c)
Observed that the floor wall junctures by the water heater are not coved. Code: Floor and wall junctures shall be coved.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee phone and belongings stored with and above food and food contact surfaces. COS. Removed. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
