Observed that employees are washing their hands at a hand sink that does not have hot water, so they are unable to follow proper cleaning procedures such as using running, warm water. COS. Other hand sink will be used. Code: When washing hands, proper hand washing procedures shall be followed, including using warm, running water.

Observed employee who was working with food, went to the register to take an order, was on their phone, and then went to working with food without washing their hands. COS. Employee washed hands. Code: Prior to working with food, hands shall be properly washed.

Observed that the facility does not have verifiable means of showing employee health reporting requirement acknowledgement. Code: Facility shall have verifiable means of showing employee health reporting requirement acknowledgement.

Observed employee drink in a single-use container with a lid but no straw. COS. Removed. Code: Employee drinks shall be in a single-use cup with a lid and a straw and shall be handled to prevent contamination of food and food contact surfaces.

Observed that the hand sink in the main kitchen area does not have hot water. Code: Hand sinks shall be installed to provide hot water of at least 85 F that can be tempered.

Observed that neither sink in the kitchen had hand drying provisions; the hand sink in the back did not have any paper towels and the hand sink in the front was jammed. COS. Papertowels placed at hand sink by inspector. Code: Hand drying provisions shall always be available.

Observed that the hand sink in the rear of the facility was blocked by a shop vac. COS. Moved. Code: Hand sinks shall be kept clear and accessible and only used for washing hands.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed that the facility does not have a written plan nor the items for a vomitous and diarrheal response. Code: Facility shall have a written plan for responding to vomitous and diarrheal events and shall have all the items necessary for that plan.

Observed food stored uncovered (peppers on the line, not in-use). COS. Covered. Code: Food shall be stored covered.

Observed that the rear door does not close tightly when self-closing. Code: Exterior doors shall be self-closing and tight-fitting.

Observed that the facility has an electric insect control device (bug zapper) that is directly over single-use items. Code: Insect controls devices shall be installed to prevent dead insects from coming into contact with food or food contact surfaces.

Observed that the faucet of the 3-compartment sink had a leak and that one of the spray hoses was in a state of disrepair. Code: Plumbing system and fixtures shall be maintained in a state of good repair.

Observed food scoops handle were stored in the food. COS. Scoops was removed from food. Code: Scoops with handles shall be stored in a manner that prevents the handle from contacting the food.

Observed containers stacked while wet. Code: Containers shall be thoroughly air dried prior to stacking.
17A - toilet facilities: properly constructed, supplied, cleaned

Observed there was no lid on top of the staff restroom trash can. Code: Bathrooms used by women shall have a trash can with a lid for sanitary products.

Observed that the food service permit was not displayed. Code: The food service permit shall be displayed in public view.
