Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed that the facility was unable to show in a verifiable manner evidence of employee health reporting requirement acknowledgement for all employees in the food service area; manager stated that the employee in the food service area is one of the corporate people and that they don't usually work here which is why they do not have a form. Recommended action: Discussed that if employees will be handling food in the food service areas then they need to have verifiable means of showing employee health reporting requirement acknowledgement or that the corporate could be restricted from entering the food service areas/handling food. Code: Facility shall have in a verifiable manner evidence of employee health reporting acknowledgement.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloth not stored in sanitizer solution on the counter and then a wet wiping cloth that is placed folded over the side of the sanitizer bucket but not stored in it. COS. Wet wiping cloths placed into sanitizer bucket. Code: Wet wiping cloths shall be stored in sanitizer solution that is at the appropriate concentration between uses.

Observed employee belongings stored over food in the reach-in cooler in the upstairs kitchen. COS. Moved. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
