Observed single service containers being used/re-used as topping scoops. Do not re-use single service items. Provide proper scoops with handles by 5/24/26.
15C - nonfood-contact surfaces clean

Observed accumulations of grease, oils, food particles and/or debris on wire rack shelving, handles of equipment, paper towel dispenser, and WIC door/handle. Clean surfaces at a frequency to prevent accumulations. Correct by 5/24/26.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulations of oils dust, and debris on floors under, walls behind and ceilings over work areas especially hand sink, prep tables/stations and make line. Also observed missing cove base under hand sink and under pizza box racks. Repeat items. Need to replace missing ceiling tile over mop sink and rear door. Correct by 5/24/26.
