Observed expired serv-safe certificate posted (expired 9/2024). No other employee certified. CA: PIC will ensure that there is a CFSM or equivalent at facility. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals.

Observed cream cheese in reach in fridge held at greater than 41 degrees Fahrenheit. CA: Told PIC to discard cream cheese. All other items held in fridge are shelf stable. PIC put in work order to have cooler fixed.

Observed food debris build up on shelves and floors under coolers. Observed dust build up on fans. CA: Engage in regular cleaning of facility.
