Observed pizza stored atop prep counters after employee moved to another task, not covered or protected from contamination.

CA: Parchment paper was placed on pizzas.

Observed ceiling tiles with mold like substance on them in the employee restroom.

Observed a chemical bottle stored on a shelf above facility food (oil) and above facility to-go lids. Observed chemicals store don a storage rack, above clean dishes.

CA: Chemicals were removed and rearranged by PIC.

Observed RTE NON-TCS food items stored/prepped in the facility for over 24 hours, not date labeled. Items were labeled in the Walk In Cooler, but not in the Prep Cooler.

CA: Items were properly dated by PIC.

Observed a dead roach on the wall behind the inspection report.

CA: PIC removed dead roach, discarded it and washed their hands afterwards.

Observed multiple TCS food items stored on the floor throughout the kitchen area:

1. Pizza dough on the floor in prep area.
2. Pizza sauce on the floor in the Walk In Cooler.
3. Boxes containing food product stored on the floor in the Walk In Freezer.

CA: Items were placed on racks and crates that were at least 6 inches from the ground.

Observed employee drinks/food stored above facility items in the Walk In Cooler. Observed employee cellphone stored under the dough mixer. Observed employee food/drinks/purse stored atop a prep counter in the back of the kitchen.

CA: Items were removed by the PIC and placed in a designated area.
