Observed residue on lid and walls inside ice machine in front desk area. CA: Food contact surfaces shall be clean to sight and touch

Observed 2 spray bottles of blue liquid without a label on the container of its contents. CA: Any spray bottle used in cleaning a tourist room shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

Observed hot water in rooms reaching temperatures above 120 degrees F. CA: Hot water must be maintained below 120 degrees F

Facility unable to present a Gas-Fire Inspection report. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed clean dishes stored in rooms for next guest's use. PIC stated dishes are washed rinsed and sanitized in 1 compartment sink in the kitchen of the room and then washed rinsed and sanitized again in dishwasher in the room. Observed Dishwasher maximum temperature at 130.2 degrees F CA: Dish machines used to clean dishes between guests must have the ability to sanitize dishes or 3 compartment sink must be installed for the purpose of cleaning dishes

Observed plates bowls in rooms stored upright without protection from contamination. CA: Food utensils and equipment shall be stored in a manner to avoid contamination COS: PIC inverted dishes

Observed rust/orange residue inside bottom of toilet and pink residue on shower head nozzles (See below) CA: Bathing facilities must be maintained clean and in good repair

Observed housekeeping cart on 3rd floor with 3 water bottles reused and filled with lotion, soap, body wash with no labels and container of toilet cleaning solution with toilet scrubber and no labels. Observed 4th floor blue liquid in a bottle with no label CA: Chemical containers must be labeled with the name of its contents

Observed dead roaches, dead and live lady bug in room 406. CA: Pest must be controlled to eliminate presence in facility. COS: Cook's Pest control treated during inspection

Observed clean dishes stored in rooms for next guest's use. PIC stated dishwasher used to clean dishes. Observed Dishwasher maximum temperature at 142.8 degrees F and not equipped with a sanitizer or sanitizing cycle CA: Dish machines used to clean dishes between guests must have the ability to sanitize dishes or alternative way to sanitize dishes must be used

Observed mattress cover stains on beds in room 422 and 424. Observed damaged mattress pad in room 408. CA: Linens must be maintained clean and in good repair

Observed rust/debris residue on shower head nozzles of room 422. Observed pink residue on shower head nozzles of rooms 409, 408 and 406. Observed chipped in room 409. Observed slight/small tub shower stains in room 422 CA: Bathing facilities must be maintained clean and in good repair
