6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed the following foods cold holding greater than 41 degrees F: Tomatoes in prep top cooler in main kitchen; ranch in ice bath in front service area; ranch, lettuce, shred cheese, pico de gallo in walk in cooler near dish area in main kitchen. CA: TCS foods must be cold held at 41 degrees or below. COS: PIC discarded the food, and moved remaining TCS foods in walk in cooler to the beer cooler.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(4)(a)(b)(c)(e)(f) - responsibility of permit holder, pic, and conditional employees (p), (pf)
The PIC could only provide and an unsigned employee health agreement. CA: The PIC must provide documentation or demonstrate that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. PIC had currently present employees sign the policy during inspection. All employees must sign a copy/signed copies must be made available at the facility within 10 days.
2-2B - proper eating, tasting, drinking, or tobacco use

Observed employee eating an apple in a food preparation of main kitchen. CA: Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. COS:PIC had employee put food away
