Observed air dryer in women's restroom malfunctioning. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with:

1. Individual, disposable towels; Pf
2. A continuous towel system that supplies the user with a clean towel; Pf
3. A heated-air hand drying device; Pf or
4. A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.Pf PIC placed paper towels inside women's restroom.

Observed dead cockroach inside storage room. CA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. PIC will have dead pests removed.

Observed no hand drying provisions at employee handwashing sink. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: 1. Individual, disposable towels; Pf 2. A continuous towel system that supplies the user with a clean towel; Pf 3. A heated-air hand drying device; Pf or 4. A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Pf PIC will provide paper towels for handwashing sink.

Observed an unapproved grill on the premises that is unnecessary to the operation of the establishment. CA: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter. PIC will remove items from the premises. Repeat Violation.

Observed holes in the back of door in the men and women's restroom. CA: All physical facilities shall be maintained in good repair. PIC will repair doors.

Observed ice scoop in bin of ice not stored with handle above product. CA: During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container PIC stored ice scoop properly.

Observed an accumulation of trash around the outdoor dumpster. CA: Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil or becoming attractants for insects and rodents. PIC will have the area cleaned.

Observed hole in men's restroom door. CA: All physical facilities shall be maintained in good repair. PIC will have door fixed.

Observed dispensing utensil in bin of ice not stored with handle above product. CA: During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container PIC stored the utensil with handle above products.

Observed single service items not stored in plastic to prevent contamination. CA: 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. PIC stored single service items in plastic.
