Observed cheese and pasta in the WIC not covered. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC covered the food items.

Observed the equipment in the kitchen not free from an accumulation of food debris. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will have the equipment cleaned.

Observed the floor in the kitchen not free from an accumulation of food debris. CA: Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the floor cleaned.

Observed the ceiling tiles in the kitchen area not free from an accumulation of dust. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the ceiling tiles cleaned.

Observed employee hand washing sink missing paper towels. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with individual, disposable towels. PIC placed a roll of paper towels at the sink.

Observed the facility had no written consumer notification of the presence of major food allergens in unpackaged food items. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. PIC was provided with a copy of the Food Allergens and Allergen Awareness handout.

Observed the floor in the kitchen not free from an accumulation of food debris. CA: Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the floor cleaned.

Observed the coving missing along the wall behind the toilet in the women restroom. CA: In food service establishments in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved. PIC will have the coving replaced.

Observed the ice machine not free from buildup. CA: Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency necessary to preclude accumulation of soil or mold. PIC will have the ice machine cleaned.

Observed no cold water at the bar handwashing sink. CA: Water under pressure shall be provided to all fixtures, equipment, and nonfood equipment that are required to use water except that water supplied as specified under subsection (1)(k)1 and 2 to a temporary food service operation or in response to a temporary interruption of a water supply need not be under pressure. PIC will have the water repaired.

Observed the ceiling tiles in the kitchen not in good repair. CA: All physical facilities shall be maintained in good repair. PIC will have the ceiling tiles replaced.

Observed several holes in the wall near the soda machine. CA: All physical facilities shall be maintained in good repair. PIC will have the holes repaired.

Observed coving missing along the wall near the WIC. CA: In food service establishments in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch (1 mm). PIC will have the coving replaced.
