Observed live small flies near mechanical dish machine. CA: he presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed door panels of prep-top-coolers and single door reach-in-cooler are unclean to sight and touch. Observed shelves inside walk-in-cooler unclean with debris. Observed floors in walk-in-cooler #1 & #2 unclean with black debris. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Observed toilet facility unclean. CA: Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
