Observed multiple live flies in the main kitchen near the trash can and near the handwashing sink. Observed damage to floor in main kitchen near the back door, creating an entry point for pests. Observed one dead roach in the seating area and one dead roach near the restrooms. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises Discussed with PIC that they must repair the damage to the floor to close off the gap from the back door to the outdoors. Pest control will be added. Pest control has come out to spray, but they did not leave a completed work order.
