Observed a plant sitting in the hand-washing sink upon arrival CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. A handwashing facility may not be used for purposes other than handwashing. COS: Food employee removed the plant from the handwashing sink.
12A - contamination prevented during food preparation, storage, display

Observed two bags of napkins sitting on the floor in the main kitchen. CA: Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, at least 6 inches (15 cm) above the floor.

Observed a leak in the pipes under the 3-compartment sink in the main kitchen. CA: A plumbing system shall be repaired according to law; P and maintained in good repair.
