Observed facility is conducting the pest control by in-house non-licensed professional.
CA: Discussed with person in charge that pesticides shall only be provided by a licensed professional
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Observed facility doesn't have recent annual gas fire inspection report. Last gas fire inspection report was from 4/22/24.
CA: Discussed with the person in charge that all gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Observed the computer chairs in rooms damaged and/or worn out. These rooms included #133, #167, #263, #279, #255, #241, #235, #208, #222, #230
CA: Discussed with the person in charge that all furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Observed permit was not posted due to nonpayment.
CA: Discussed with the person in charge that any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. Facility must pay for TA permit by correction date of 1/20/26.
The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away
