Observed shredded mozzarella cheese, butter milk and cabbage stored inside the beer walk-in cooler holding at temperatures above 41*F. CA: Advised PIC Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS: PIC discarded food items during time of inspection
4-2B - food-contact surfaces: cleaned & sanitized

Observed the side table can opener stored unclean with food debris present along the metal blade. CA: Advised PIC equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC placed the can opener in the 3-compartment sink to be washed and sanitized

Observed pizza racks unclean with heavy build-up. CA: Advised PIC equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Rack placed inside the 3-compartment sink

Observed 4 knives stored as clean with food debris on the magnetic strip and food debris on the magnetic strip. CA: Advised PIC Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC placed inside the sink to be washed and sanitized
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed the beer reach in cooler measuring 43F ambient unable to hold TCS food items at 41F or below. CA: Advised PIC . Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. CA: PIC has been advised to provide an update with when the temperature has been lowered and provide a photo via email of the internal thermometer, PIC has been advised to discontinue storing TCS foods until repaired
15B - warewashing facilities: installed, maintained, used; test strips

Observed incorrect test strips for testing Quat sanitizer and no test strips for chlorinated dish machine. CA: Advised PIC A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided.
15C - nonfood-contact surfaces clean

Observed black/white substance along the green racks inside the beer walk-in cooler and rusted racks inside walk-in cooler #1. CA: Advised PIC Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues
15C - nonfood-contact surfaces clean

Observed the walk-in cooler fans unclean with heavy debris build-up. CA: Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed heavy dust debris along the rack in front of the prep top cooler. CA: Advised PIC nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the floors unclean and with food debris inside the walk-in freezer. CA: Advised PIC The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
