Observed cold holding TCS (Temperature Controlled for Safety) foods above 41°F at the salad bar. See temperature log for said items. TCS food must be cold held at 41°F or below. If the ham had been thawed earlier it should not have been put on the salad bar until it is 41°F or below. COS: CFSM discarded the ham.

Observed uncovered slices of cake on a rolling rack in WIC. Several slices of orange and lemon cake were uncovered. Slices of cake not protected/covered from sources of contamination. COS: CFSM covered all of the slices of cake.

Observed wet wiping cloths laying on food prep tables and not stored in sanitizing solution between uses. COS: Employee removed all wet wiping cloths and put in sanitizing solution.

Observed soiled nonfood contact surface of knife holder over three compartment sink, inside cold holding units, hot holding units, shelves, etc. PIC will have everything cleaned within three to five days.

Observed soiled food contact surface of Manitowoc automatic ice machine. Observed pink and black mildew inside of the unit. It will be cleaned today.

Observed soiled nonfood contact surfaces of plate warming box, rolling cart, shelves Carter Hoffman warmer, rusted shelves in Delfield cooler, and dirty, soiled food storage containers (oatmeal and grits). Will have everything cleaned in five to seven days and will start on getting the shelves in cooler repaired/replaced. Tables in dining room are stills sticky.

Observed soiled nonfood contact surfaces of metal shelves in kitchen area. Observed the tables in the dining room are very sticky. While writing up the report, my arms kept sticking to the table. Will have shelves cleaned within three to five days and address the dining room tables within two to three weeks.

Observed soiled floors throughout kitchen area: debris under the shelves and soiled walls in the storage room area with dry goods. The walls will have to be repainted. Floors and walls will be taken care of within seven to ten days.
