Observed PIC not providing documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: All employees shall complete an employee reporting agreement. COS: Gave copy of agreement to complete.

Observed person-in-charge (PIC) not knowing the location of the employee reporting agreement or its contents. CA: PIC shall be aware of the employee health policy to ensure no employees are working while sick. COS: Gave a copy of employee health policy.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed no paper towels at hand wash sinks in customer and employee restrooms. CA: All hand wash sinks shall be supplied with a hand drying provision. COS: Paper towels placed sinks.

Observed time/temperature control for safety foods found stored past disposal date marked. CA: All date marked TCS foods shall be discarded once the date mark has passed. COS: Foods discarded.

Observed food items in storage containers were not identified with the common name of the food item. CA: All food not stored in original container shall be labeled with common name.

Observed packaged food in the store not labeled as specified by law. CA: All packaged food shall be labeled with establishment name, address, phone number, food ingredients if more than one ingredient and allergens.

Observed time/temperature control for safety (TCS) foods thawed at room temperature. CA: All TCS foods shall be thawed using one of the approved methods outlined in regulations. COS: Food placed under cold running water.

Observed wet wiping cloth not stored in sanitizing solution between uses. CA: Moist wiping cloths shall be stored in sanitizer solution not soapy water between uses.

Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee personal items stored in multiple locations throughout kitchen. CA: Personal belongings shall be stored in a designated area away from food, clean equipment and single service/use items. COS: Personal items moved.

Observed trashcans with no lids in employee and customer restrooms. CA: Replaced trashcans with ones with lids.
