Observed no physical copy of an employee health policy. CA: . The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee. PIC will be given another copy of the employee health policy form for employees to sign today (4/10/2024).

Observed PIC not able to provide documentation of an employee health policy. PIC was given a copy of an employee health policy today (08/23/2023) that all employees will read and sign.

Observed missing drain plug on outside dumpster. PIC will have until 09/28/2023 to have drain plug put on outside dumpster.

Observed visible mold on ceiling tile over cabinets located over 3 compartment sink. PIC will have until 09/28/2023 to have ceiling tile over cabinets located over 3 compartment sink replaced.
