4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed in the main kitchen in the reach in cooler:
- Container of raw chicken stored above mozzarella cheese and raw whiting fish.
- Box of raw eggs stored on the top shelf above cooked beans, refried beans and containers of cooked beef.
- Container of raw eggs stored behind cooked collard greens and American cheese.
- 2 containers of raw chicken touching 2 containers of raw whiting fish.
- Ground beef stored above raw whiting fish. CA: Raw animal foods must be separated according to minimum required cook temperature and stored in a way to prevent cross contamination of food. Ready to eat foods must be stored away from raw animal foods to prevent contamination. COS: PIC stored raw animal food items on the bottom shelf not touching each other.

Observed a container of flour being used for both flour and chicken in the main kitchen on the countertop. CA: There must be a separate container of flour for chicken and fish. Raw animal foods must be separated according to minimum required cook temperature and stored in a way to prevent cross contamination of food. COS: PIC created two separate containers of batter labeled for fish and chicken.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(a) - handwashing cleanser, availability (pf)
Observed two hand sinks, one in the front food service area and one in the main kitchen, without a supply of hand soap. CA: All handwashing stations must have hand soap provided. COS: PIC provided liquid hand soap at both handwashing stations.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed opened cooked chicken sausage and containers of chopped red onion and margarine stored uncovered in the reach in cooler in the main kitchen. Observed 2 bags of rice, a bag of batter, and a bag of corn flour stored uncovered in the main kitchen on the food rack. CA: All food items must be stored covered to protect from potential contamination. COS: Food items were covered by food employee and PIC.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(g) - ready-to-eat time/temperature control for safety food, date marking (pf)
Observed refried beans, cooked beans, 2 containers of cooked beef, cooked shredded chicken, containers of salsa, collard greens, coleslaw mac & cheese, cut lettuce and sliced American cheese opened and held for 24hrs or cooked and held for over 24 hours in reach in cooler without a date label. CA: All TCS food items that have been opened or prepped and held for over 24 hours must be properly date labeled. COS: PIC and food employee placed the prep dates (4-12 for the pork, 4-11 for all other items) on all required food items.

Observed sauce bottles (cream, butter, and salsa, etc.) unlabeled in the main kitchen reach in cooler. Observed sauce bottles (salsa, hot sauce, bbq) unlabeled on the counter in the front food service. Observed containers of black beans, shredded chicken, salsa, collard greens, coleslaw, and more all unlabeled in the reach in cooler of the main kitchen. CA: All food items taken out of its original packaging shall be provided with a label with its appropriate name written. COS: PIC and food employee wrote appropriate names on the food containers.
