Observed no screens in windows of rooms 116, 120, 205, 333, and 432. C/A: Openings to outside shall be protected against the entry of rodents and insects through tight-fitting doors, closed windows, screening, or other means

Observed cracked window in room 205. C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed lack of anti-slip material in bathtubs of rooms 120, 116, 205, and 333. C/A: Anti- slip tubs slip strips or slip proof mats shall be made available in each bathing facility and be kept clean and in good condition

Observed stains on sheets, pillows, pillowcases, and mattress/bedbug covers in rooms 333, 331, 410, and 430. C/A: Bedding and linens should be cleaned and sanitized

Observed walls in the hallways with missing baseboards and holes. C/A: Walls in hallways and rooms should be clean and in good repair

Observed lack of anti-slip material in bathtubs of rooms 333, 410, and 430. C/A: Anti-slip strips or slip proof mats should be provided in each bathing facility

Observed last gas fired equipment inspection done 5/11/2023. C/A: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed broken handle in the room 227. C/A: All furnishing shall be maintained clean and in good repair
