- Observed stain on sheets in room 106.
- Observed hair on sheets in room 212.
C/A: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
17 - proper ventilation; no sign of mold, mildew, odors or moisture
Regulation: 511-6-2-.08(5) - toilet and bathing ventilation
Observed bathrooms with no ventilation in rooms 106 and 402.
C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed fallen debris from ceiling on floor in room 106. PIC mentioned possible water damage from kitchen above room may have led to damage.
C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored
