Facility does not have a verifiable manner in which shows that employees are trained on their responsibilities regarding the 5 symptoms and 6 illnesses regarding food illnesses C/A Reporting Responsibilities. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food;Pf COS EHS left employee health policy with the manager to be copied and had ever employee sign it

Observed a dead American Cockroach/palmetto bug laying on its back in the lobby area. C/A Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

Eliminating harborage conditions. COS Facility has 10 days to correct the violation
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed plastic trays wet stacked on the shelf across from the ware washing machine in the main kitchen. C/A Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils:

Shall be air-dried or used after adequate draining before contact with food; and

May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
COS Manager unstacked the trays and allowed them to dry
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf)
Observed facility does not have any strips available to test the Quat at the sink and chlorine at the warewashing machine C/A Sanitizing Solutions, Testing Devices. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Pf COS Facility has 10 days to correct the violation and email the EHS to update the inspection report
