3-1C - food in good condition, safe, and unadulterated
Regulation: 511-6-1.04(3)(e) - package integrity (pf)
Observed a large can of aloe jelly with jagged/chipped metal edges on the can opening, creating the potential for metal shards to contaminate the product. C/A: Food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. COS: Discarded
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed black container completely blocking hand sink in ware wash area. C/A: A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: Removed.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed paper towels were not available at hand sink (middle main kitchen). C/A: Each handwashing sink or group of adjacent handwashing sinks shall be provided with:
- Individual, disposable towels;
- A continuous towel system that supplies the user with a clean towel;
- A heated-air hand drying device; or
- A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. COS: Napkins provided
12D - washing fruits and vegetables
Regulation: 511-6-1.04(4)(g) - washing fruits & vegetables (pf)
Observed food employee rinse limes off at the 3 compartment warewashing sink of main kitchen. C/A: Raw fruits and vegetables shall be thoroughly washed in water, in a sink designated for that purpose only. COS: PIC informed employee on where to wash produce.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed flying insects through out main kitchen. C/A: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and
