TCS food items stored inside the walk in cooler was observed maintaining cold holding temperatures above 41F. Observed food stored inside the walk in freezer was not maintained frozen. CA TCS food items must maintain cold holding temperatures at 41F or below and stored frozen to prevent foodborne illnesses. COS TCS food items were discarded and transferred into the prep units stored on the front prep line and kitchen where temperatures were maintained at 41F.

A mouse was observed peaking his head through the small hole in the wall of the mop sink. The mouse quickly disappeared inside the wall. CA Holes must be sealed inside the facility to prevent rodents, insects and or bugs from entering inside the facility.

An egg beater was observed stored behind the 3-compartment sink faucet directly touching the wall where mold was observed. CA Utensil must be stored in areas that will prevent contamination. COS The utensils were removed.

COS Ice was stored on TCS food items that were prepped for 30 minutes, TCS food items were discarded and or stored inside the prep unit on the front prep line until the unit is repaired.

Sanitizer buckets were observed not stored on the prep line inside the kitchen. CA Sanitizer buckets must be available on prep lines to prevent contamination. The concentration of the sanitizer must test at 50ppm for bleach or 200ppm for QAC. COS The sanitizer bucket tested at 50ppm for bleach solution. A pink substance was observed at the top of the ice machine. CA Equipment must be sanitized to prevent contamination. COS An employee is in the process of sanitizing the ice machine.

CA The 3-compartment sink must be set up in the correct order to follow the labels listed on the sink to ensure dishes are being properly sanitized. COS The sink was drained and set up in the correct order.
