Upon arrival, observed food employee wash has in cold water. Advised PIC food employees shall use the following cleaning procedure in the order stated to clean their hands and exposed portions of their arms, including surrogate prosthetic devices for hands and arms: rinse under clean, running warm water. COS: Had food employee wash hands in warm water for 20 seconds.

Observed no key drop delivery contract upon request, per definition of key-drop for after business hours or when facility is closed. Advised PIC monitoring of receiving during Non-Operating Hours. Employees are verifying that foods delivered to the food service establishment during non-operating hours are from approved sources and are placed into appropriate storage locations such that they are maintained at the required temperatures, protected from contamination, unadulterated, accurately presented, by routinely monitoring the employees' observations, maintaining receiving/corrective action records for deliveries during non-operating hours and periodically evaluating foods upon their receipt as specified within DPH Rule 511-6-1-.04(3)(m).

Observed visible light entry at back door of facility. Advised PIC outer openings of a food service establishment shall be protected against the entry of insects and rodents by: solid, self-closing, tight-fitting doors.

Observed multiple back packs stored on top of pizza boxes. Advised PIC lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. COS: Had PIC relocate personal bags.

Observed no key drop delivery upon request. Advised PIC Key-Drop deliveries are not being monitored as per 511-6-1-.04(3)(m).

Observed expired vomit/fecal response dated: 10/20. Advised PIC Responding To Contamination Events. A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.

Observed accumulated debris on fan non-food contact surfaces inside of Walk-In-Cooler. Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed encrusted food debris on the surfaces of the can opener and a lid in the dish washing area stored with clean utensils. Advised PIC all food contact surfaces shall be cleaned to sight and touch.

Observed multiple sauce bottles in the prep top cooler without labels. Advised PIC all bulk items placed in containers shall have a label.
