Observed stains on mattress, mattress pads, and pillows in the following rooms: 126 (pillow), 238 (Mattress protector), 325 (Mattress Protector), 319 ( Mattress Protector), 317 (Mattress), 313 (Mattress). Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed no anti-slip material in bathtubs in rooms 116, 132, 238, 325, 319, 317, 313. Advised PIC Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. Advised PIC to place anti-slip strips, appliqués, or slip-proof mats in all guestroom bathtubs

Observed visible light entry at the entry door of guestrooms in rooms 116, 120, 126, 330, 332, 334, and 336. Advised PIC openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Advised PIC to place weatherstrip/seal to all guest room entrance doors

Observed no anti-slip material in bathtubs in rooms 116, 120, 124, 126, 330, 332, 334, 336. Advised PIC Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. Advised PIC to place anti-slip strips, appliqués, or slip-proof mats in all guestroom bathtubs

Observed no gas-fired equipment inspection report on file at time of inspection. Advised PIC Advised PIC All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority

Observed debris in dress drawers (room 222, 330, 320); rusting in microwave for room 22. Advised PIC all furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed no anti slip in tubs for rooms: 336, 330, 220, 222, 338, 238. Advised PIC anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observed gas fired equipment inspection report dated: 8/16/23. Advised PIC All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed stains, hairs behind headrest for rooms: 126, 128, 134, 334 (stains on pillow), 332 (stain on mattress), 328 (hole in mattress cover). Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed debris on vent in rooms: 128, 134 (stains on vent), 338, 334 (minor debris), 332, 328 (stains on vent). Observed debris on showerhead unit: 126, 128, 134, 328. Advised PIC toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed guestroom water temperatures exceeding 120 degrees F. in rooms 224 (125F), 122 (126F), 226 (128F), 120 (121F), and 330 (126F). Advised PIC that all guestroom water temperatures shall not exceed 120 degrees F. Advised PIC to lower guestroom water temperatures

Observed staining on mattress pad, mattress cover, and mattress in guestrooms 218, 224, 334, and 216. Also observed tear on mattress pad in rooms 224 and 226. Advised PIC Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Advised to clean and replace bedding in guestrooms

Observed staining and debris on bathroom vent in rooms 218, 120, and 332; observed rusting near bathtub drain in rooms 330 and 216; observed debris in tub in rooms 224 and 332; observed staining on bathroom towels in room 224' Observed residue on showerhead in room 330. Advised PIC Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. Advised PIC to thoroughly clean bathrooms in rooms listed

Observed ice machine panel and the 1st and 3rd floor stored as clean with black/brown mold-like substance. Advised PIC that all ice machines shall be kept clean at all times. Advised to have both ice machines serviced

Advised PIC Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. Advised to place anti-slip appliance to all bathtubs

Observed housekeeping carts with multiple chemical bottles unlabled with a common name. Advised PIC any spray bottle used in cleaning rooms or stored on house-keeping cart shall be labeled with its contents. CA: PIC has 72 hours to fix and provide evidence to health authority. 4th repeat
