Observed facility without an allergy awareness training. Food employees shall be properly trained in food safety, including food allergy, as it relates to assigned duties. Stated to PIC to obtain and train all food employees on allergy awareness.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed multiple sauce bottles on counter near grill prep area in the main kitchen and PTC in the main kitchen stored with food products unlabeled. Working containers holding food or food ingredient that are removed from its original container shall be clearly and legibly identified in English with the common name of the food. Stated to PIC to label all sauce bottles with common name.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(k) - insect control devices (c)
Observed insect control device stored above the hand sink near the back door of the main kitchen. Insect control devices that are used to electrocute or stun flying insects shall be located/installed so that the device is not located over a food prep area and dead insects and insect fragments are prevented from contact with exposed food, clean equipment, utensils, linens. and unwrapped single-service/single-use articles. Stated to PIC to relocate insect control device.

Upon arrival, observed the 2025 Food Service permit posted in public view. A current Food Service Establishment permit shall be posted in public view. Stated to PIC to post the 2026 Food Service permit in public view.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed multiple food contact surfaces of the exterior of toaster. exterior of grill, and interior of PTC with excessive accumulation and debris. Nonfood contact surfaces of equipment shall be kept free of accumulation of dust, dirt, food residue, and other debris. Discussed with PIC cleaning schedule of nonfood contact surfaces.
