Upon Arrival, observed handsink blocked by multiple containers of soap and bleach obstructing. All handsinks shall be maintained so that it is easily accessible at all times.

Observed handsink without disposable towels at the sink. CA: each handwashing sink shall be provided with individual disposable towels. See Base of Operation.

Observed no written notification of food allergen. Permit holder shall notify written notification of the presence of major food allergens.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(l) - removing dead or trapped birds, insects, rodents, and other pest (c)
Observed dead insects on the floor in the main kitchen and front customer service. CA: dead insects and other pests shall from control devices and the premises that prevents their accumulation. See Base of Operation.

Observed no current inspection report posted at food facility. CA: The most current inspection shall be prominently displayed in public view at all times. See Base of Operation.

Observed broken tiles in main kitchen of food facility. CA: All physical facilities shall be maintained in good repair. See Base of Operation.
