Observed PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibility to report symptoms and illnesses of foodborne diseases. Advised PIC that an employee health agreement form(s) shall be produced upon request of the Health Authority. Advised PIC to obtain signed copies of employee health agreement forms.

Observed multiple sauce bottles located on the prep table in the main kitchen not labeled with common name. Advised PIC label information shall include the common name of the food. Advised PIC to label all sauce bottles with common name.

Observed Provisional permit that is expired posted in public view. Advised PIC to post the permit in a location in the food service establishment that is conspicuous to consumers. Advised Pic to obtain a permanent 2026 Food Service Establishment permit and post in public view.

Observed incorrect inspection report posted at time of inspection. Advised PIC The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. Advised PIC to post the most recent inspection report in public view.

Observed cardboard lining shelving above crock pot. Advised PIC nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. COS: PIC removed cardboard lining.
