Observed facility without a food allergy awareness training during time of inspection. Advised PIC that the facility shall have a training regarding food allergy awareness. Advised PIC to obtain/create food allergy awareness training and provide to the Health Authority upon request. (From Base of Operation Inspection: 11/25/2025)
2-2A - management knowledge, responsibilities, reporting

Observed PIC could not demonstrate in a verifiable manner that food employees are/were informed of their responsibilities to report foodborne diseases and symptoms. Advised PIC that an employee health policy shall be provided to the Health Authority upon request. Advised PIC to obtain an employee health policy and provide to the Health Authority upon request. 16A - hot and cold water available; adequate pressure

Observed hot water not available on mobile unit. Advised PIC that hot and cold water under pressure shall be available at all times. COS: PIC repair hot water on mobile unit.
13A - posted: permit/inspection/choking poster/handwashing

Observed 2025 Food Service Permit not posted in public view. Advised PIC that a current Food Service Permit shall be posted in public view. Advised PIC to obtain 2025 Food Service Permit and post in public view.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed nonfood contact surfaces of RIC #3 and RIC#5 with excessive food debris. Advised PIC that all nonfood contact surfaces shall be cleaned. Advised PIC to thoroughly clean all nonfood contact surfaces. (From Base of Operation Inspection: 11/25/2025)
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed personal cell phone on prep table in the main kitchen. Also observed personal drink on table storing clean blender sat the front-service area. Advised PIC that all personal belongings shall be a designated area. Advised PIC to place all personal belongings at the designated area. (From Base of Operation Inspection: 11/25/2025)
