Observed TCS food items held at front service prep top cooler measuring above 41 degrees F. Advised to discard. CA: All TCS food items shall be cold held at 41 degrees F or below.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed no signed employee health reporting agreements. Advised to obtain. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. 2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed no vomitus/fecal cleanup procedure at facility. Advised to obtain. CA: All food service facilities shall have written vomitus/fecal cleanup procedure. CBD: 11/15/25.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(c) - storage, separation (p)
Observed can of bug spray stored on shelf with syrup, straws, and chia seeds. Also observed bottle of Windex stored with nozzle facing straws, waffle bowls, and nuts. COS: PIC moved items. CA: Toxic materials shall be properly stored to prevent cross contamination.
10D - food properly labeled; original container
Regulation: 511-6-1.04(8)(b) - expired foods (p)
Observed jugs of expired milk in front service reach in cooler. COS: PIC discarded. CA: Use in date products.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed insufficient hot water at hand wash sinks, prep sink, and 3-compartment sink. Facility must voluntarily closed until sufficient hot water is restored. CA: Hand wash sinks shall have hot water minimum 85 degrees F and prep sinks and 3 compartment sinks shall have hot water minimum 110 degrees F. CBD: 11/15/2025.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed ice cream scoops stored in stagnant water. Advised to get running water working. CA: In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes. CBD: 11/15/2025
