Observed no food allergen awareness training for employees. Also, Observed no key-drop contract on file. Employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction. Employees are visibly observing and verifying delivered foods as they are received to determine that they are from approved sources and are placed into appropriate storage locations, as required by this Chapter, such that they are received and maintained at the required temperatures, protected from contamination, unadulterated, and accurately presented, by routinely monitoring the employees’ observations, maintaining receiving/corrective action records for deliveries during non-operating hours and periodically evaluating foods upon their receipt as specified within DPH Rule 511-6-1-.04(3)(m).

Upon Arrival, observed no CFSM posted. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. COS: Had PIC post CFSM in public view.
4-2B - food-contact surfaces: cleaned & sanitized

Observed food contact surface of multiple lids stored as clean on storage rack with stains. Equipment food-contact surfaces and utensils shall be clean to sight and touch.
15C - nonfood-contact surfaces clean

Observed nonfood contact surfaces of shelving racks in the walk-in cooler, shelving near 3-Compartment Sink, and interior of prep area reach-in-cooler with accumulation of debris in need of cleaning. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
