Observed TCS foods cold holding on a cart above 41F. Advised PIC TCS foods shall cold hold at 41F or below. Advised to re-locate to walk in cooler.
2-2A - management knowledge, responsibilities, reporting

Observed Facility with written agreement for employees to report food borne illness/ symptoms. Advised PIC to obtain agreement forms, have employees sign and keep readily available upon Health Authority's request.
4-2B - food-contact surfaces: cleaned & sanitized

Observed black substance on surfaces of the soda nozzles in the dining area. Advised to clean.
