Observed deli slicer stored as clean but visibly dirty along posterior side with old food. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS by cleaning.

Observed black mold in bulk ice machine. Ice machine must be emptied, cleaned, and sanitized within 72 hours. Must increase cleaning frequency to a rate that precludes mold accumulations.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed very large opening at bottom of back door. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors.
Must add door sweep to door to prevent entry of pests.

Observed lid of bulk ice machine in disrepair. Also observed doors of 3-door reach-in cooler not properly closing and being left ajar. Must fix both.
