Observed ice dispensing equipment that allows contamination of the lip-contact surface discolored with pink/brown mold like buildup. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Observed a large amount of unused equipment throughout the facility. Observed ice machine to be clean on the inside but have a heavy hard water stain on the front of the unit from constant leaking. Observed front right reach-in cooler not capable of maintaining proper cold-holding temperatures. RCA: Front cooler is utilizing time as public health control due to equipment incapable holding proper cold holding temperature of 41F or below. Observed ice buildup in the walk-in freezer. Several boxes of food (garlic bread and pizza) had water damage and a significant build-up of ice on them. Advised PIC that food should not be stored under the fan in the freezer and that the items that are affected need to be thrown out.
RCA: Package Integrity. Food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.

Observed salad prepared in house for self-service stored in cold holding unit with ambient temperature of 60F. Internal temperature of salad measured at 62F.
CA: Prep started at 0945 and salad put on line at 1030. PIC advised food needs to be tossed at the end of lunch service 1255pm.
RCA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F.

COS CA: PIC properly reheated food items to 165F and will monitor temperatures. TCS food items shall be held at 135F or above unless actively cooling.

Observed accumulation of matter on baffle of ice machine.
RCA: Equipment, food-contact surfaces, and utensils shall be clean to sight and touch and cleaned and sanitized more frequently.

RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed milk being held in cold-holding unit not capable of maintaining temepratures of 41F or below. Milk is pulled out for 20 minutes, so still in SAFE zone. RCA: TCS food items shall be held at 41F or below constantly.

Observed hot holding warming cabinets not capable of maintaining temperatures of 135F or above. Food within cabinets were around 120F. CA: PIC properly reheated food items to 165F and will monitor temperatures. RCA: TCS food items shall be held at 135F or above unless actively cooling.

Observed a large amount of unused equipment throughout the facility. Observed ice machine not in good repair. RCA: PIC indicated that the ice machine has a work order in to be repaired. PIC provided a list that was submitted to the school board of the equipment that is not usable and needs to be removed from the facility.
