Observed prep cooler not holding temperature. All items in prep cooler were placed in unit at 11:00am. At 3 pm, temperature was 55 degrees F. All food product in unit was discarded (Brussel sprouts, tomatoes, green onions, bang-bang sauce etc.) See temperature log for specifics. Facility to utilize additional coolers until repairs are made.

No sanitizer present in sani-buckets. PIC removed all buckets and remade fresh to correct concentration.

Observed employee wearing bracelet while prepping food. PIC had employee remove bracelet. Educated PIC on only being allowed tov wear a plain wedding ring. 511-6-1.03(5)(g) - Jewelry (C) (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

-Observed employee cell phone stored on top of prep table in main kitchen. Purse stored on top of dry goods in bar area. Educated PIC on proper storage of employee items. Items must be stored under and away from food product, single services items and clean dishes.
